Begin with the new patient request process. Requests are reviewed before scheduling is finalized.

Existing Patients
Established patients may use the secure portal to view appointments, complete forms, access billing information, and receive practice communications.
Portal access is intended for established patients or patients who have received portal access during the request process.

Patients confirm their physical location at each visit. Treatment cannot be provided when a patient is outside the physician’s licensed jurisdictions.
